Care Management Service:

• in-home assessment of the person’s emotional, social, and financial needs
• individualized action plan to address the unique issues of the person
• Organize and coordinate service delivery systems
• Monitor and modify services, as the person's needs change over time
• Assist with alternate living arrangements, if it becomes necessary

Outreach Services:

• Assist individuals to complete application forms for available services including applying for benefits such as Old Age Security and Canada Pension Plan
• Resource information on various services such as housing, health services, transportation and advocacy/legal
• Referral service to appropriate community organizations, government agencies and other organizations that offer support and assistance to seniors and families
• Assist families and supportive others to search for elder care alternatives
• Act as a liaison to family members at a distance

Telephone Reassurance and Support:

• Provide personal daily phone contact with seniors to help link them to the community, encourage social contact and independence

Home Support:

• Trained, caring individuals can assist with activities of daily living
• Provide emotional and physical support
• Assist seniors with services such as gardening, lawn mowing, pick-up and delivery of prescriptions and health care aids
• Help prepare grocery lists, read flyers and comparison shop, shop for or with seniors, help deliver groceries

Get-Away Club:

The Get-Away Club is a day service for persons with a moderate level of cognitive impairment. Services include structured daily activities for clients, respite for caregivers, solution-focused counseling and support to caregivers, educational support, and client assessment at home or at the Centre.
